Responsibilities

Aspire Health is seeking a social worker who will be responsible for providing psychosocial support to patients and their families via telephone and/or video as a part of an innovative new program Aspire to Live Better. The Aspire to Live Better social worker will work closely with patients and families who are nearing the end of life, and are often at high risk for re-hospitalization due to complex illness, family dynamics, and other barriers to care. Aspire to Live Better’s social workers are skilled in providing comprehensive supportive care to these patients and their families, and facilitating discussions about end of life planning and care.

 

Ideal candidates will have experience identifying and coordinating the needs of chronically ill patients and families as well as supporting clinicians to help them operate as efficiently as possible. Responsibilities will be highly varied, and appropriate candidates will be multitalented, flexible problem solvers who are eager to tackle complex problems and tasks. 

  • Provide critical psychosocial assessment and support to patients and families identified to have complex Palliative Care needs
  • Provide education about disease progression, care options and expectations, and advance care planning
  • Serve as a liaison with the patient’s managing medical team, the Aspire support team, and other community based resources including hospices, nursing homes, and home health providers to ensure care coordination and appropriate resource allocation to meet patient needs throughout the continuum of care
  • Facilitate formal and informal meetings regarding patient and family communication and decision making
  • Assist patient’s medical team and families to clarify goals of care and plan of care that meets the unique needs of each patient
  • Document social work assessments, care plans, and interventions
  • Coordinate grief support services when appropriate
  • Provide community resource and service referrals as needed

Qualifications

  • Master’s degree in Social Work required
  • Must have active license to practice in the state of residence
  • Minimum of 3 years social work experience required
  • Prior healthcare experience serving chronically ill, geriatric populations preferred
  • Health plan, home health, or hospice experience a plus
  • Strong customer-service orientation
  • Patient, compassionate and professional demeanor required
  • Exceptional organizational skills and ability to multi-task
  • Self-starter with an entrepreneurial spirit and demonstrated problem solving skills
  • Strong writing, Word, Excel, and database management skills
  • Extremely strong work ethic
