    Aspire Health is the nation’s largest community-based palliative care provider. Aspire specializes in providing an extra layer of support to patients facing a serious illness through an ambitious palliative care program focused on caring for patients in their homes and outpatient clinics.  Aspire’s clinicians are experts in providing patients with relief from the symptoms, pain, and stress of a serious illness; helping patients and caregivers navigate the healthcare system; guiding patients and caregivers through difficult and complex treatment choices; and providing emotional and spiritual support to patients and their families.

     

    In collaboration with Aspire’s local Lead Physician, Nurse Practitioners develop complex plans of care in accordance with the patient’s health status and overall goals and values; provide medical diagnostics and treatments in the home consistent with the nurse practitioner’s scope of practice and the patient’s medical condition; educate patients and families on medication usage, diet and nutrition, as well as crisis anticipation and prevention; and communicate medical information to the patient’s physicians and other providers as well as to Aspire’s Lead Physician and broader clinical team. In addition to patient care, the Clinical Manager will serve as the primary clinical mentor / coach to their assigned teams (comprising 5-8 other APPs and SW team members). The envisioned split is estimated to be an average of 50% clinical care /50% Clinical Manager responsibilities with variation based on market and team needs. They will support the Clinical Director in assuring team orientation, competence, professional development and team integrity. The Clinical Manager APP will take part in weekly interdisciplinary team meetings, call schedules and scheduled in-service and education sessions. They will role model Aspire core values and serve as the exemplar clinician in the field.

    Responsibilities

    • Under the direction of the Clinical Director or Regional Vice President of Clinical Operations, serve as the supervisor to assigned clinical teams: mentoring, providing ride along training, supporting professional development plans and assessing clinical competence.
    • Collaborate with the assigned lead physician to address the support needs of the assigned market(s) and clinical staff.
    • Develop and implement clinical plans of care for adult patients facing chronic and life-limiting conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions)
    • Gather history and physical exam and diagnostics as needed in the home setting. Then develop and implement treatment plans given patient’s goals of care and current conditions
    • Meet patient and family physical and psychosocial needs with support and input from inter-disciplinary team
    • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention
    • Communicate with other Aspire clinical team members, patients’ other physicians and patients’ other medical providers to coordinate optimal care and resources for the patient and his or her family
    • Communicate with Aspire’s local Lead Physician to review patient records and care plans on a timely basis consistent with state regulations and Aspire Health standards and policy
    • Maintain patient medical records and medical documentation consistent with state regulations and Aspire Health standards and policy
    • Participate in continuing education as required by the state Board of Nursing
    • Prescribe medication as permitted by the state Board of Nursing
    • Other related duties as deemed necessary

    Qualifications

    • Minimum of 3 years’ experience as a (geriatric, homecare, or hospice and palliative) APP OR at least a year tenure with Aspire exhibiting field leadership skills.
    • Must have excellent clinical outcomes.
    • Must have demonstrated ability to lead nurse practitioners / physician assistants.
    • Must maintain a good performance evaluation for candidacy and maintenance of title.
    • Enthusiasm and compassion for patients and their families, along with a consistent positive attitude about their role in the organization.
    • At least 1.0 FTE.
    • Candidates must have demonstrated ability to collaborate with and motivate a team to higher levels of performance.
    • Must have RVP-Clinical Operations, RVP-Operations, lead physician(s), Chief Clinical Officer and EVP/Market President approvals for promotion or hire into the Clinical Manager role.
    • Family or Adult Nurse Practitioner with appropriate certification and licensure required.
    • Possession of DEA registration or eligibility required.
    • CPR certification required.
    • Must have reliable automobile, valid driver’s license and minimum state required liability auto insurance; nurse practitioners travel in personal vehicle with reimbursement for mileage.
