    Our Nurse Practitioners (NPs) play a vital role at Aspire – and this is an ideal choice for the individual who has the passion, devotion, and desire to impact the lives of the patients and families we serve.  Our NPs collaborate with our Lead Physicians, the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values.  To support our nurse practitioners, Aspire also has a centralized Patient Care Center, which provides a team of social workers, registered nurses and non-clinical staff that support our nurse practitioners.  In addition, our NPs take part in weekly interdisciplinary team meetings, call schedules and scheduled in-service and education sessions.

    Responsibilities

    • Provide palliative care and chronic care management services in an assisted living facility, independent living facility and/or skilled nursing facility
    • Perform initial and follow up exams with patients to develop and implement clinical plans of care for adult patients in conjunction with a collaborating Aspire physician and patients’ primary care physicians
    • Seek to meet patients’ and families’ physical and psychosocial needs with support and input from the Aspire interdisciplinary team
    • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention
    • Collaborate with other Aspire clinical team members, patients’ other physicians and patients’ other medical providers in a timely manner that is consistent with state regulations and Aspire Health standards and policy
    • Maintain patient medical records and medical documentation consistent with state regulations and Aspire Health standards and policy through an EMR system
    • Participate in continuing education as required by the state Board of Nursing
    • Prescribe medication as permitted by the state Board of Nursing
    • Participate in call schedule
    • Perform other related duties as deemed necessary

    Qualifications

    • Compassion for our patients and their families, and understand how our delivery model meets the needs of so many individuals
    • Advanced nursing degree in adult, family, acute care or geriatric medicine
    • Experience in an assisted living facility, independent living facility and/or skilled nursing facility caring for a senior patient population a plus
    • Experience in palliative care and/or chronic care management a plus
    • Possession of DEA registration or eligibility required
    • Excellent oral and written communication skills to ensure that interactions with patients, family caregivers, clinicians, and team members are effective and meaningful

    Knowledge/skills/abilities:

    • Palliative care and chronic care management knowledge and expertise a plus
