Ideal candidates will have experience with telephonic identification and coordination of needs in chronically ill patients and families, as well as providing clinical and administrative support to providers. Responsibilities will be highly varied, and appropriate candidates will be multitalented, flexible problem solvers who are eager to tackle complex problems and tasks.
Telephonic care coordination with palliative care patients and their families, including incoming calls about acute issues as well as proactive outgoing calls to build rapport with patients telephonically while educating on disease process, conducting goals of care conversations, and addressing acute issues. Assess, identify and close clinical and non-clinical gaps in patient care
Appropriate triage and escalation of routine, urgent and emergent patient needs.
Clinical care coordination with PCP’s and other providers and specialists, including but not limited to:
A successful candidate will have an active RN licensure (required). Minimum of 3 years clinical experience required, preferably in a multidisciplinary team environment. Prior healthcare experience serving chronically ill patients (preferred). Fluent in Spanish (preferred, but not required). Telephonic case management or care coordination experience a plus.
